About Face Surgical Arts strives to make outstanding care affordable for our patients.
Personalized treatment plans are created for every patient to ensure they experience an optimal result. The cost of your individual treatment is dependent on several factors, such as insurance coverage, type of oral surgery procedure, and overall treatment plan.
If you are covered by insurance, you are responsible to pay the balance as determined by your insurance plan. Some procedures offered may not be covered by your insurance.
Payment is due at the time of service. We accept several forms of payment and work with many insurance companies to achieve treatment at a reasonable cost. We accept payment via cash, check, VISA®, MasterCard®, American Express® and Discover®.
About Face Surgical Arts works with CareCredit to serve your financing needs. CareCredit is a credit card designed to help provide a way to pay for treatments and procedures immediately.

Insurance Information
We are considered in-network with the following Dental Insurances:
- Aetna
- Anthem (Complete, Prime, Blue, Grid)
- Cigna
- Delta Dental PPO
- Delta Dental Premier
- Dental Care Plus

We are considered in-network with the following Medical Insurances:
- Anthem PPO
We opt out of Medicare-Medicaid, so you are entering a private contract of self-pay.
Being out-of-network does not always mean you do not have benefits. We ask that you contact your insurance company to see what your out-of-network benefits are. We strive to help you make optimal use of your dental insurance, and as a courtesy to our patients, we are happy to file all your dental claims.
Your insurance is a contract between you, your employer, and your insurance company. With the complexity of insurance policies and the constant changes made by insurers and employers, keeping up to date is challenging. Neither employers nor insurance companies notify us of any changes. Insurance companies have decided that it is the member’s responsibility to know their insurance limitations.
Please make sure that you have given us accurate insurance information at least 2 days prior to your appointment. If we do not have your current information, the claim will be rejected, and unfortunately, you are responsible for the balance.
Questions you should ask your insurance:
- If I am out-of-network, what is my benefit level?
- What services are covered?
- Do I have a co-pay or deductible?
- Has the insurance carrier, group number, or address changed?
Not only do all the insurance companies differ in requirements and coverage, but their policies and procedures constantly change.
